VEHICLE OWNERSHIP AND REPAIR AUTHORIZATION
I certify that I am the owner of the vehicle identified above or am authorized by the owner to approve any services or repairs that my vehicle needs.
I authorize Access Collision Center LLC to inspect, diagnose, and perform repairs on my vehicle as separately approved by me. I understand that additional repairs or charges may require my authorization before work proceeds.
VEHICLE OPERATION AUTHORIZATION
I authorize Access Collision Center LLC and its employees to operate, move, and test-drive my vehicle when reasonably necessary for inspection, diagnosis, repair, or quality-control testing.
REPAIR COMPLETION AND INSURANCE SUPPLEMENTS
I understand that repair completion dates are estimates and may change due to parts availability, insurance approvals, additional damage discovered, repair supplements, or circumstances beyond the repair facility’s reasonable control.
PERSONAL BELONGINGS AND VALUABLES
I acknowledge that I am responsible for removing all personal belongings and valuables from my vehicle before leaving it with Access Collision Center LLC, including cash, jewelry, electronics, documents, and other personal property.
I understand that Access Collision Center LLC is not responsible for personal belongings left inside the vehicle that are lost, stolen, or damaged, except where responsibility cannot legally be excluded.
PAYMENT RESPONSIBILITY
I understand that I am responsible for all repair charges I have authorized, including amounts not paid by my insurance company and any applicable insurance deductible, subject to applicable law and any separate written agreement.
CUSTOMER ACKNOWLEDGMENT AND AUTHORIZATION
By signing below, I acknowledge that I have read and understand this Authorization to Repair agreement and agree to its terms.
